Developed by Dr. Barry Reisberg, the Global Deterioration Scale (GDS) outlines the progression of dementia in seven stages. Understanding these stages can help you prepare and provide the best possible care for your loved one.

 

Stage 1: No Cognitive Decline

At this stage, your loved one appears normal and shows no signs of cognitive impairment. However, subtle changes may be occurring in the brain, making them appear to “think slower”.

 

Stage 2: Very Mild Cognitive Decline

Occasional memory lapses become noticeable, such as forgetting familiar objects or names. These symptoms are often mild and may not affect daily life significantly.

 

Stage 3: Mild Cognitive Decline

Cognitive impairment becomes more apparent. Your loved one may:

 

  • Get lost in unfamiliar places
  • Forget recent events or names
  • Struggle with complex tasks
  • Feel anxious about their symptoms

Stage 4: Moderate Cognitive Decline

Symptoms are now evident in clinical interviews. Your loved one may:

 

  • Struggle with current events or personal history
  • Have trouble organizing or managing finances
  • Avoid challenging situations

 

Stage 5: Moderately Severe Cognitive Decline

Your loved one will likely need assistance with daily tasks. They may:

 

  • Forget important details like their address
  • Become disoriented with time and place
  • Need help with decision-making

 

Stage 6: Severe Cognitive Decline

At this stage, your loved one requires significant care. They may:

 

  • Forget the names of close family members
  • Experience emotional changes like paranoia or agitation
  • Need assistance with daily living activities

 

Stage 7: Very Severe Cognitive Decline

In the final stage, your loved one will need full-time assistance with all personal care tasks. They may lose motor skills and the ability to communicate effectively.
